Below is information on the many options available to obtain your sports physical for the 2026–2027 school year. Completed physical forms must be submitted to the Ottawa High School office by Friday, August 14th, as the first day of fall practices is Monday, August 17th. We encourage you to get your physical at the start of the year if you plan to play a sport at any time during the 2026-2027 school year.
